I am not expecially convinced that it is possible to frame a disclosure statement so that it is more useful than what we have now. That said, even if we could write such a n informing document, it gets us away from what should really happen: there ought to be a full-blown home inspection, not some paper-based substitute completed by people who may be both honest and uninformed.
